From the IWF 2023 Anti Doping Rules
12.3.2 Multiple Anti-Doping Rule Violations by Athletes or Other Persons affiliated to a Member Federation

Should three (3) or more violations of these Anti-Doping Rules sanctioned by IWF or Anti-Doping Organizations other than the Member Federation or its National Anti-Doping Organization have been committed by Athletes or other Persons affiliated to the Member Federation within a 12-month period, the
Independent Panel may, after taking into account both the seriousness of the underlying anti-doping rule violations and the gravity of the circumstances
surrounding the case:

a) impose Member Consequences on the Member Federation of a period of up to (4) years; and/or

b) fine the Member Federation up to $500,000 USD to be paid within 6 months from the receipt of the Independent Panel’s decision. If the
Member Federation fails to pay the fine within such deadline, further Member Consequences for an additional period of up to two years, or, if earlier, until the fine is settled in full, may be imposed by the Independent Panel on the Member Federation concerned. For the avoidance of doubt, the fine remains due to IWF after the further Member Consequences have been fully served.

I believe that it is still detected via blood. The last few years a blood spot analysis has been developed - who knows if it was what picked up these cases.

AFAIK DBS sampling/analysis has mostly made it much cheaper/practical to run blood testing, so more tests are getting done. 4 April suggests it was an in-competition test. Given the detection window of hGH with DBS sampling, that has some interesting (amusing?) factual implications.

List of athletes with 3 or more tests. As far as I remember, all of those >3 tests were Olympians. All Olympians tested at least once.

Many of those with 2 tests were either Olympians or North Korean, with some strays for good measure.

Many Olympians tested once (most likely at the Olympics?). Multiple test athletes are mostly juniors from World Junior Championships.
Can't be bothered processing the results.
